9/12/08 - Temple University registration is CLOSED.  Please do not fill out the survey below if you are trying to register with Project SHINE in Philadelphia.

---------------------------------------------

Before embarking on the submission of your Student Intake Form, please take a moment to answer the following survey questions.

We at the national consortium have designed a short survey to better understand the body of students that support SHINE work, both at the start of their semesterly service and at the end of their SHINE experience. The purpose of this information is strictly used for our own organizational development. Thank you for taking the time to complete this survey!

If you do not have a student ID number, please enter the last four digits of your social security number.
